QlikView AccessPoint shows No Server and logs read Account: Failed to create account IQVS_Servername, Access is denied.

Description
After installing QlikView using IIS, AccessPoint gives you the error message "No Server". This happens even though everything is running and and QMS shows green on all the services.
The Windows event logs shows the following error:
300 Error Account: Failed to create account IQVS_xxxxxxx, Access is denied. (Error Code : 5 (NERR_BASE + -2095))
By default the QlikView service account is not running the identity for the QlikView IIS application pool.
Resolution
The service account running QlikView must be used as the IIS Application Pool identity.
1. Open the IIS manager 2. Select Application Pools on the left 3. Right click QlikView IIS 4. Select Application Pool Identity default 5. Scroll to Application Pool Identity and click on the eclipse to edit 6. Select Custom and enter the QlikView service account and password. 7. Restart the application pool
